Between 9/26 and 11/30 you can simply purchase $15 worth of the above pictured products and submit for a $5 rebate in the form of a Walmart gift card, while supplies last. The rebate form says receipt (not plural) so I am not sure if you can combine purchases. I would suggest purchasing all in one transaction if you plan on doing this rebate.
